The 2022 CT5-V with the 3.0L twin-turbo V6 (LGY engine) is still relatively new, but early examples are showing concerning patterns of catastrophic internal engine failures and transmission cooling issues that suggest fundamental design or manufacturing problems requiring vigilance.
Catastrophic Engine Failure - Connecting Rod Bearing Failure
Occasional · high severity
Typical onset: 15,000-50,000 mi
Symptoms: Sudden onset of heavy knocking or rod knock sound from engine, Metal shavings in oil during routine change, Loss of oil pressure warning, Engine seizure or complete failure while driving
Fix: Complete engine replacement or full rebuild required. Rod bearings fail prematurely, destroying crankshaft and block. 25-35 hours labor for replacement, often covered under powertrain warranty if still active. Out-of-warranty means used/reman long block or complete tear-down and machine work.
Estimated cost: $12,000-18,000
Transmission Oil Cooler Leak
Common · medium severity
Typical onset: 30,000-60,000 mi
Symptoms: Transmission fluid puddle under vehicle front-center, Low transmission fluid warning light, Burnt transmission fluid smell, Harsh shifting or slipping if fluid level drops significantly
Fix: External cooler lines or cooler itself develops leaks at fittings or seams. Replacement of cooler assembly and lines, 3-4 hours labor. Requires fluid refill and system flush. May cause secondary transmission damage if driven low on fluid.
Estimated cost: $800-1,400
Transmission Mount Failure
Occasional · medium severity
Typical onset: 40,000-70,000 mi
Symptoms: Clunking noise during acceleration or deceleration, Excessive vibration at idle in gear, Shifter movement or harshness, Visible sagging or torn rubber on mount during inspection
Fix: Transmission mount separates or tears, especially on hard-driven examples. Replacement requires supporting transmission, 2-3 hours labor. Check all engine mounts at same time as they share similar design.
Estimated cost: $400-700
Piston Ring Failure and Cylinder Scoring
Rare · high severity
Typical onset: 20,000-60,000 mi
Symptoms: Excessive oil consumption (1 quart per 1,000 miles or worse), Blue smoke from exhaust on startup or acceleration, Loss of compression in one or more cylinders, Misfires and rough running, Oil fouled spark plugs
Fix: Piston rings fail or cylinders score due to manufacturing defects or inadequate break-in oil film. Requires complete engine disassembly, cylinder boring/honing, new pistons and rings. 30-40 hours labor. Often accompanies bearing failure diagnosis.
Estimated cost: $10,000-16,000
Fuel Filter/Pump Module Contamination
Occasional · medium severity
Symptoms: Rough idle or hesitation under load, Intermittent no-start or extended cranking, Loss of power at higher RPM, Fuel pressure fluctuation codes (P0087, P0088)
Fix: In-tank fuel pump module filter screens get clogged with debris from tank or fuel system. Requires tank drop, module replacement. 4-5 hours labor. Often related to poor fuel quality or contaminated fuel from station.
Estimated cost: $900-1,500
Head Gasket Failure (Secondary to Overheating)
Rare · high severity
Typical onset: 40,000-80,000 mi
Symptoms: White smoke from exhaust, Coolant loss with no visible external leaks, Oil contamination with coolant (milky oil cap), Overheating episodes, Misfires and rough running
Fix: Head gaskets fail typically after cooling system issue causes overheat event. Both heads on V6 require removal, resurfacing. 18-24 hours labor. Check for warped heads requiring machine work. Often discover additional problems once disassembled.
Estimated cost: $4,500-7,500
Owner tips
Change oil at 5,000 mile intervals maximum with high-quality synthetic - engine bearing failures are widespread enough to warrant religious maintenance
Monitor oil consumption closely - any usage between changes is red flag for ring or bearing issues on this engine
Check transmission fluid color and level every oil change - cooler leaks are common and silent killers
Verify any used example has complete service records and oil analysis history if available - bearing failures often give advance warning in oil samples
Avoid extended warranty gap - these engines fail young enough that factory powertrain warranty status is critical
Inspect engine mounts and transmission mount during every service - aggressive driving accelerates failure
Skip it unless under full factory warranty - the LGY engine's catastrophic bearing and internal failures at low mileage represent unacceptable risk for used buyers, and repair costs exceed many vehicles' total value.

General Motors, LLC (GM) is recalling certain 2020-2023 Cadillac CT4 and CT5; 2021-2023 Buick Envision; and 2022-2023 Cadillac Escalade and Escalade ESV, Chevrolet Silverado 1500, Suburban, Tahoe, GMC Sierra 1500, Yukon, and Yukon XL vehicles. The daytime running lights (DRLs) may not deactivate when the headlights are on. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 108, "Lamps, Reflective Devices, and Associated Equipment."
Consequence: DRLs that do not deactivate as intended may result in a glare, reducing visibility, and increasing the risk of a crash.
Remedy: The body control module software will be updated by a dealer, or through an over-the-air (OTA) update, free of charge. Owner notification letters were mailed January 23, 2023. Owners may contact GMC customer service at 1-888-988-7267, Chevrolet customer service at 1-800-222-1020, or Cadillac customer service at 1-800-458-8006. GM's number for this recall is N222386380. This recall expands previous recall number 22V-827.
